ConnectWELL - Social Prescribing Project ConnectWELL is a way to improve your life your way. The aim is to identify what matters to a person and then link an individual to a range of non-medical community activities and organisations to help improve Health and Wellbeing such as: advice and information services community groups leisure activities lunch clubs self-help groups specialist interest groups sporting activities and lots lots more. ONE quick easy referral form enables people to have a consultation with us to signpost them to as many of the 1200+ activities and services available in Rugby as would help them to improve their Health and Wellbeing.
For the social prescribing service self-referrals are accepted and all medical practices across Rugby can refer patients. Referrals can be made via crccg.connectwell@nhs.net If you wish to refer yourself please click here or call 01788 539572. After referral: ConnectWELL Link Workers can support you to identify and access the activities that interest you ConnectWELL Health Buddies can offer support and encouragement to attend the signposted services Activities we signpost to: If you are a Voluntary Organisation or Community Group and would like to tell us about the activities that you offer follow the link to our Project Directory to give the opportunity for our team to signpost people to your service. When ConnectWELL signposts a person to an activity it is the individual's choice to follow up attend and to ensure that they are satisfied that the Voluntary and Community Sector Organisation has appropriate policies and procedures in place.
