What Is Dementia Connect? Dementia Connect is a free service that provides information, support and advice to anyone living with or affected by dementia who lives anywhere in Warwickshire. Once referred into the service (either via self referral or referral from a professional) one of our comprehensively trained Dementia Advisors will make contact with the referred person who can then initiate and tailor support to the individual. Dementia Connect is made up of the following components;
Telephone Dementia Advice: The Alzheimer's Society will provide a specialised information and signposting service to anyone who needs it. This part of the service will be phone based. Advisors will provide an efficient analysis of the service user's needs and offer support. When a service user needs further support the Dementia Advisor will arrange for a face to face visit with the service user at their home or an appropriate location.
Face to Face Dementia Advice: This is an offer to anyone with more complex needs to receive a thorough assessment from a Dementia Advisor to see how they can best support the person referred and provide tailored information advice and support.
Keeping in Touch: Each service user will be offered a Keeping In Touch call a minimum of twice per year. The calls will be made by volunteers and aim to remain in contact with service users prevent crisis and ensure they are supported throughout their dementia journey. Through this offer we will ensure that we are able to meet the change of individual needs and if they require any further support we have the opportunity to link them back to the Dementia Advisor Team either face to face or over the telephone.
Self-Management: We offer a range of online resources such as Talking Point our online forum which enables peer support with people in communities across the country and a wide range of fact sheets available in different languages and formats. These resources enable people affected by dementia to remain in control of their own journey and to select the support to best meet their individual needs.
