Longhurst Group provide a short-term support service for adults aged 18 years and over who are experiencing vulnerabilities but do not meet the threshold to access statutory support from social care under the Care Act 2014 or support from other agencies according to their specific eligibility criteria. The short-term support service aims to prevent to reduce and/or delay their need for care and support. Individuals who may benefit from a short-term targeted intervention service to prevent reduce and/or delay the need for longer term care and support due to; circumstance age illness or disability; AND whose independence and well-being is or may be vulnerable to significant harm. People who may benefit from a targeted intervention service include those: presenting risky behaviours who have a chaotic lifestyle experiencing difficulties in a certain aspect of their life e.g. loss or bereavement housing issues criminal justice self-esteem self-harm who are disengaging from statutory services.
